What are markup annotations in a PDF?

Will removing markup change my text?
No. Markup overlays the text rather than modifying it, so the content is unchanged.
Why is a strikethrough still there?
It may be text formatting from a word processor rather than a PDF annotation. Edit it in the source application.

Some tools allow targeting subtypes; others clear everything. Check before running.
Does this work on scanned documents?
Only for markups added digitally. Marks on a scanned page are pixels and need image-level treatment.
Do comments attached to markup get removed?
Usually, since the popup belongs to the markup object. Verify afterwards.
Is it reversible?
Not after saving. Keep the original.

Marginally. Use compression if file size is the actual concern.
